Creating a tailored SAP Appeal form inside of your PeopleSoft instance is a breeze using XTend from Lagniappe Software.  Using pre-populated data to provide the name of the student, student ID and the SAP statuses.  Additionally, the pre-populated SAP statuses can drive the conditional logic for the form.  In this example below, the student is deficient in one area. 

The pre-populated GPA Deficiency status drives the conditional logic to provide the student with the correct information in the form. 

In this example, the student is deficient in 3 areas:

The SAP form is automatically tailored to the student based on their SAP status.  This student has multiple deficiencies, therefore, the form provides a section specific to each.

Students can also upload an attachment in any section of the form:

Forms like this can be created, tested and launched within hours using XTend.  Additionally, forms like this can be restricted and pre-assigned to students using PS Query.
